PUSH has instituted late cancellation/no show fees in order to provide the best access for all patients.
There is a $25.00 no show fee for appointments canceled less than 2 hours prior to the scheduled appointment time.You may avoid that $25.00 no show fee by canceling the appointment at least 2 hours before the appointment time.
You may pay this charge at the Bursar's office or online through your MyPurdue account.
Students are allowed a grace period of only 5 minutes for a scheduled appointment. Patients should arrive at least 15 minutes prior to the scheduled appointment to ensure you have adequate time for the check in process.
There is a high demand for PUSH services and no shows take away opportunities for other students to get care when they need it.
You may cancel appointments by calling 765-494-1700, or through the Patient Portal.
Physical Therapy appointments can be canceled by calling 765-494-1839.
To appeal the no-show fee, please complete the Appeal Form. There are two options available, an online form or a printable form.
For the printable form, complete it and either fax it to (765) 496-1907 or drop it off in the immunization box on the front of the PUSH building.
