Center for Healthy Living offers sports physicals for $25
The Center for Healthy Living on Purdue’s West Lafayette campus is available for sports physicals.
Sports physicals continue to be available for $25 for dependents covered on a Purdue medical plan. Although sports physicals are not covered under the Purdue medical plans, faculty and staff can use their PayFlex debit cards to pay for the sports physical from their health savings account (HSA), health care flexible spending account (FSA) or health care reimbursement account (HRA).
“Everyone getting a sports physical will need to bring the required IHSAA form to the appointment,” said Jen Hodges, director of operations at the center.
Call 49-40111 to make an appointment or schedule via the patient portal. If the sports physical will be the child's first visit to the center, Hodges recommends filling out the New Patient Health History Form in advance and bringing it to the appointment to help save time.
Remember, a sports physical does not replace an annual preventive exam for your child with his or her regular pediatrician. Hodges encourages parents be sure to get both for their child(ren).
The center is open Monday through Thursday, 7 a.m. to 6:30 p.m. and Friday 7 a.m. to 5 p.m. Lab hours are 7 a.m. to 4 p.m. Monday through Friday.
