NOTE: In order for your request to be accepted by the Office of the Registrar, you must send your transcript(s) for review 5 business days prior to the completion of this form.

*If inquiring about a test score, please provide any identifiers such as Batch ID, Personal Code, Cycle Number, Date printed or sent. These can be found on the receipt that was emailed to you by the testing agency.

We will expedite your credit evaluations for one of the following reasons. Please choose the one that applies:
